Arizona Department of Health Services (ADHS) houses death records from 1844 through the present year. Anyone can obtain a death certificate for a person deceased at least 50 years.

50 years ago to present year (certified certificate)
Arizona is a "closed record" state. That means that vital records are not public record. Arizona law restricts the public's access to vital records to protect the confidentiality rights of its citizens.
A person must be 18 years of age or older may obtain a certified copy of a death certificate.

- Attorney Representing an Estate
- Attorney Representing Family Members
- Attorney Representing Persons with a Legal Interest

Restrictions for Obtaining Death Records
Restrictions for persons born who died within the last 50 years:
To obtain a copy of the death certificate for those who died within the last 50 years, you must be:
- The parent or grandparent of the person on the certificate
- The spouse of the person on the certificate
- The child or grandchild of the person on the certificate
- The sibling of the person on the certificate
- Funeral director
- Anyone who has legal interest in the certificate
- NOTE: A copy of the death record cannot be obtained by anyone except those listed above until 50 years have passed from the date of the death.
