- Name of each spouse
- Occupation of each spouse
- Date and place of birth of each spouse
- Residence of each spouse
- For minors, consent or authorization must be given
- Option of "monogamy or polygamy eventually subscribed by the spouses"
- "Payment or not of a dowry under conditions of marriage"
