  • Parents' names and ages
  • Parents" profession and residence
  • Where appropriate, those of the declarant or the witnesses

"If the day of birth cannot be specified by the declarant or the witnesses, a date of birth is fixed ex officio by the Civil Registrar or by the President of the People’s Court in the village-centre or district."[2]
