Information in the Registration Affidavits generally include the following:

  • Full Name of applicant
  • Date Registration was Filed
  • Address when registered
  • Age
  • Physical description
  • Photo
  • Married or not
  • Been in Military
  • Number of Children (If married)
  • Children's Gender
  • Finger prints
