Status (single/married)
Profession
Date and place of birth
Place of citizenship
County
Residence in Ljubljana
Wife - maiden name, date and place of birth
Children - legitimacy, date and place of birth
Other household members

If there are no other people living in the household except for the head of household, the word Nikdo! or Nihče! (meaning nobody) is noted on the form.
